The Professional Partner Program's Goal is to provide and coordinate services in the community for families and their children with serious emotional disturbances. In the PPP program youth is the primary focus of support; however, services are provided with the entire family's needs in mind. Community services and individuals from the community are utilized who have skills and are willing to support the family as part of a "family team." The team decides when, where, and how much support is needed. Flexible funds are available if other resources are inadequate.

WHO'S ELIGIBLE?

Youth under 21

Persons who exhibit a significant degree of functional impairment due to serious emotional disturbances

Youth who are state wards or at risk of becoming state wards

Youth at risk of being placed out of the home

Youth at risk of dropping out of school

Youth who have committed criminal offenses

HOW DOES IT WORK?

The Professional Partner Program's primary focus is on the family. The PPP works with the families as equal partners in developing a unique individualized family service plan that addresses the needs of the family in the least restrictive, least intrusive and most normalized environment. An intake committee reviews the family's application, considers the severity of needs, and prioritizes enrollment. In partnership with the family, a community team is developed to devise an individual Family Service Plan of wraparound services.

This approach includes providing the majority of services in the natural environment of the youth and family (e.g., home, school). The Professional Partner assists the family in getting the plan to work.

WHAT IS WRAPAROUND?

Wraparound is a process of empowerment. It is a life plan developed by an individualized community team of people who know the family best. Wraparound means that the family has access and ownership of the plan. The Professional Partner Program works with the family in securing traditional and non-traditional care.

TRADITIONAL MENTAL HEALTH CARE SERVICES

In-home Family Support Systems

Outpatient therapy

NON-TRADITIONAL WRAPAROUND SERVICES

An aide to assist the family with an uncooperative child

Involvement in social/recreational activities

Tutor to help the child with school work

Job placement assistance or a job coach

Help a parent network with other families

Crisis response 24 hours a day, 7 days a week

FEES

Fees for the entire package of services will be negotiated based on the family's ability to pay.
